Come learn about Living Wills\, Powers of Attorney for Healthcare\, & POLST Forms. Complete your forms and walk out of the workshop with your executed documents in-hand\, a and a smile on your face. Workshops begin at 1 PM\, 2 PM\, & 3 PM on January 16th!\n \n\n\nThis FREE offer makes it easy to complete your advance directives without the need to hire an attorney.\nAnyone over the age of 18 can participate.
